The only thing that springs to mind is a failed moult. Or that by some chance when she last moulted there was some parasite involved in the disruption of the moult and regrowth process.

There is a very slight chance she was roughly handled when still soft eg netted and transferred to a new tank. I have seen my native shrimp with snapped off rostrums and put it down to the rostrum being caught in the net and snapping off.

To be on the safe side I would isolate this shrimp with some plants you don't mind throwing out if she doesn't survive or is contagious.

That is a genetic defect. Notice her carapace shell isn't completely formed as well (along the bottom)?

Commonly seen in shrimps that have been too inbred.

Best to cull this shrimp and ensure it doesn't breed with another shrimp.
